Direct Debit Request & Claims Benefit Form

Complete this form to pay your premiums by direct debit from your bank account and/or to nominate a bank account for your claims benefits to be deposited into.

Health Management Form (Healthier Lifestyle)

On selected nib health covers, you can claim on Gym Membership and Weight Management Program fees through your Healthier Lifestyle benefits.

Third Party Authority Form

You can choose to give any person listed on your policy (e.g. a partner, parent or child) authority to operate the policy, including making claims on behalf of anyone listed on the policy.
